This form is to schedule spay/neuter services ONLY.

All pets must be on a leash or in a carrier.

*Effective April 6th, a $30 non-refundable deposit is required for all surgeries and will be applied toward the total service cost. If the appointment is not rescheduled at least 24 hours prior to the scheduled time, the deposit will be lost.
